Setting and Access to Potential Subjects

The study will be conducted at the John Hopkins nursing organization. The organization is an acute-care hospital that provides different types of care. Lately, the rate of hospital-acquired infections (HAIs) among older adults (aged 65 years and above) has been high. Therefore, the study is proposed to find out if effective handwashing using soap and water or hand sanitizer is effective in reducing the rate of HAIs among older adults admitted to the hospital. The study involves educating nurses about the proper hand hygiene techniques (handwashing using soap and water or hand sanitizer) and then observing nurses perform the hand hygiene techniques appropriately and at the required points/time.

Informed consent will not be required since the proposed intervention involves educating nurses about the appropriate hand hygiene techniques and then observing them perform the hand hygiene techniques appropriately. Therefore, the study will not have any invasive treatments/interventions on the nurses who are the study subjects (Manti & Licari, 2018).

Timeline for the Project

The proposed project is projected to take about 3 months to implement. This is because the project does not include many facets. The nurses will be trained about proper hand hygiene for approximately 1 week. On the other hand, the observation and monitoring of the nurses performing hand hygiene during patient care will take approximately 10 weeks to assess if nurses are adhering to the proper hand hygiene measures.

The last 2 weeks will be used to analyze the rate of HAIs among the older adults admitted at John Hopkins nursing organization before and after the implementation of the project. This implies that the baseline data on the rate of HAIs in the organization during the pre-project period will be compared with the rate of HAIs post-project period.

Resources

Successful implementation of the proposed project will require several resources. One important resource is the human resource that includes the competent trainer to educate nurses about proper hand hygiene techniques during patient care. nurses are also an important resource as they will be involved in the implementation of the proposed project. Other resources include hand sanitizers, soap, and educational materials that will be needed during the training and when performing hand hygiene measures during patient care. Information technology and a training venue will also be required during the training of the nurses (Horntvedt et al., 2018).

Methods and Instruments

Data will be collected from the organizational records regarding the rate of HAIs pre-and post-project implementation. The findings will be presented using charts and graphs. More importantly, a questionnaire tool will be developed to assess the knowledge level among nurses regarding hand hygiene pre-and post-project periods. The collected data will then be analyzed to compare the rate of HAIs and nurses’ knowledge about hand hygiene techniques pre-and post-project periods.

The Process for Delivering the Solution

This is a multi-faceted project and thus the first step will be to identify and appraise evidence about hand hygiene techniques. The second step will include educating nurses about hand hygiene techniques (washing hands with soap and water and hand sanitizer). The nurses will be trained in the conference room in the organization and the training is estimated to take about 2 weeks. The training will occur between 3 pm-5 pm, where the first lot will be trained during the first week, and the last week the second week.

Nurses will then be observed to assess if they are performing the appropriate hand hygiene practices as required. Lastly, the baseline rates of HAIs among older adults admitted to the hospital will be compared with the HAIs rates after the implementation of the project.

Data Collection Plan

As aforementioned, data will be collected from the organizational records before and after the project is implemented. The data will include the rates of HAIs among the older adults admitted to the hospital. This is to get the rate of HAIs in the organization before and after the implementation of the project. The process will also include observing nurses performing hand hygiene techniques to assess their competency and adherence to the required hand hygiene measures.

It will be important to maintain the confidentiality and privacy of the collected data. The project manager will have the role of maintaining privacy/confidentiality and managing the collected data (Noroozi et al., 2018).

The collected data will then be analyzed using an excel spreadsheet to examine the rate of HAIs among the older adults before and after the implementation of the project and also to assess the competency/knowledge and adherence of nurses to the required hand hygiene practices.

Strategies to Manage Barriers, Facilitators, and Challenges

Various strategies will be utilized to manage any obstacles and challenges that may hinder the effective implementation of the proposed project. The first strategy will involve using change champions to educate nurses in the organization about the importance of appropriate hand hygiene measures and the efficacy of hand hygiene practices in reducing the rates of HAIs. This will be done to influence and change the attitudes of nurses towards hand hygiene in order to reduce their resistance to the proposed change (Li et al., 2019). The second strategy involves the allocation of adequate resources to implement the proposed project and convincing the management/administration about the importance of the proposed project to garner the management’s full support.

Feasibility of Implementation Plan

Albarqouni et al (2018) provide that a feasible implementation of a proposed evidence-based project analyses competency, skills, knowledge, and the involved costs. Competency, skills, and knowledge will indicate that nurses are well-prepared to adapt the evidence-based change. The costs that the project will incur are justifiable since individuals who will train nurses about the proper hand hygiene measures will be compensated. Other costs include costs to purchase educational materials, hand sanitizers, and soap that will be used in educating nurses as well as during the implementation of the proposed project. The involves costs are justifiable are they are basic for the successful implementation of the project. Moreover, the organization will save healthcare costs after the reduction of HAIs among the older adults admitted to the hospital.

Plan to Maintain, Revise, Extend or Discontinue

The team involved in the implementation of the project will examine the competency and adherence of nurses to perform hand hygiene practices as well as the effectiveness of the project in reducing the rates of HAIs among older adults as intended. If it is determined that the proposed project was not effective in reducing the rates of HAIs (highly unlikely), the project will be stopped in order to review it. There will be continuous training of nurses on hand hygiene practices to ensure they are updated about the best-available and latest hand hygiene practices. It is not envisaged that the project’s implementation period will be extended since the current plan is deemed to be adequate. Any need to extend the implementation period will be reviewed when implementing the project.
